Yes the cover says I have enclosed a copy of your signed credit agreement,The enclosed copy states under declaration that it is a application and it only has one signature. the only details telephone number and annual income. Many thanks.

I applied to Monument for a copy of my credit card CCA, have just recevied today a copy of my origanal application form with terms and conditions. What action is now open to me? Any replies will be most helpfull.
