Make a letter of formal complaint. State the problems (with details, dates etc), request a copy of complaints procedure and how to escalate to the Independent case examiner. Make a formal request, under the data protection act for ALL data - written, computerised, telephone and anything else. Follow the complaints procedure. Then contact the Independent Case Examiner and explain what the problems are and what course of action you have taken to resolve the problem. The CSA made a right hash of my case. ICE stepped in, produced a 14 page report and upheld the vast majority of my complaints of how the CSA mishandled my case. I'm ready now for making a subsequent ICE complaint as the CSA never learn and just keep on messing things up.
