Advice needed please!!!

 

I'm going to send my AQ and court fee off to my local county court today. Should I send a copy of the AQ to my bank also?

Does anyone have Lloyds TSB Solicitor's details? Are they the same for every case? I'm at work and without the details and want to post this today as deadline is the 16th July.
