A colleague was unable to attend an investigatory meeting due to having to go to the local A&E with a stomach problem. Our employer has however asked for documentary evidence that this visit took place, however enquiries to the A&E dept suggest that he would have to sumbit the request in writing and it could take up to 14 days to process.

Does anyone know if there is an official form for this sort of thing? He just needs something to confirm he attended and received treatment.

Why can't he wait the 14 days?

The employer is being rather impatient. They don't seem to accept that A&E departments don't 'admit' people and therefore don't give discharge slips etc

In his case, the employer would not have any other choice but to wait for the document... if he needs proof of his visit to the local A&E...
