We forgot to photocopy out daughters DLA form before we sent it off, its been seven years since then, will they still have the forms and can I request they send me a copy back for future reference? My daughters has learning disabilities, can I request them on her behalf, does she need to sign the letter I send?

 

Anybody requested their old files before? Thanks!

You can request the files if they still have them, they will send you copies. You can only request them if your daughter is a child. If she is an adult, you can only request them on her behalf if you are registered as her appointee. Otherwise, she will have to sign the request, or sign a letter requesting you to act on her behalf in relation to her claim as a representative.
