You send each bank a letter each. The addresses to send them to are in each of the subforums dealing with each bank/building society. I would take some time to read through the FAQ's http://www.consumeractiongroup.co.uk/forum/faqs-please-read-these/ before reclaiming your charges.

HAVE YOU BEEN TREATED UNFAIRLY BY CREDITORS OR DCA's?

BEWARE OF CLAIMS MANAGEMENT COMPANIES OFFERING TO WRITE OFF YOUR DEBTS.

In addition to what Rory has said I would also start a thread in each of the banks forums as the banks handle this in different ways to each other and you will get more specialised help in the right forum.

Good luck

BEFORE starting your claim read through the FAQ's and if there's something you aren't sure of then ask.
