No need to write, simply phone customer services and tell them you are giving them one months notice that you intend to quit the service. Done it multiple times in the past. Be wary though, they'll put you through to retentions and try tempting you to stay with super offers and stuff.

If you want to keep your number you will need to request a PAC code when you ring up. You then give this code to your new network and your number goes over in about a week and then the old phone is cancelled.

If you don't want to keep your number then Parva is more or less bang on. Though some networks (off the top of my head CPWO2 being main one) will only accept a cancellation request in writing. If you are just cancelling and not porting I'd recommend writing to your network and sending it recorded delivery. Letters have been known to "go missing" or phone calls have been known to "never have taken place"

If you are leaving and NOT taking your number elsewhere - DO WRITE by Recorded Delivery, at the same time advise you will be cancelling your DD after the final bill is calculated. Then, if something goes wrong, you have paperwork to support your actions.
