Once ended, either party can modify the agreement - which usually means it either continues unchanged, or the customer is given notice of an increase - this can be by bill stuffer, or by being noted on the bill itself. The customer has to be given 30 days notrice of the change, which ties in with the 30 days notice of an intention to cancel - you can exit with no problem before the new charges apply. (And have your old number transferred ('ported' in the parlance) to a new provider).
If they have not provided notice of the change, your GF can challenge the new fee, but they more than likely did, and the information was missed. In this case, give immediate notice of termination, or request a PAC code to release the number (which signifies the same thing).
