    Default MCOL help needed please

    Hi everyone couldsomeone help with 2 questions about filling in my MCOL please
    1) What address do i put on the MCOL ive seen lots of different addresses on these threads.
    2)What do i put in the claiment name bit as the a/c is in joint names but my husband has been doing all the leters as his better at it than me and his been signing the letters from him
    3) Last question does it make a difference to getting a result if no interesticon is asked for in the MCOL .
    any help would begreatly appreciated
