Question re Statement Of Evidence / Witness Statement

This is my first post although I've been picking up info here for a while, thanks! Have already succesfully got full settlement of charges with 8% interest and costs from Sainsbury's Visa (£500) and HSBC (£1400).
But am also claiming £2.5k from Natwest who are taking longer to crack. They have offered £1.4k which I declined, and it has now got to the stage of judge's directions. The part of the directions which has got me a little confused reads as follows:
1. The claimant must by 12 Feb 2007 serve on the defendant and file with the court signed statements setting out the evidence of all witnesses (including himself) and of all documents, including relevant bank statements on whom/which he intends to rely. His statement must identify all charges which he is seeking to challenge, must state why they are contrary to the UCTA 1977 s4 and the UTCCR 1999 para 8 and sched 2(1)(e) and why they are unreasonable.
The only bit of this that has me confused is - does this mean I write a "statement setting out the evidence" which lists my Witness Statement as a seperate document along with all the other documents (which my Witness Statement will also refer to), or do I just write one long Witness Statement which refers to all the other documents?
Obviously the info I provide will be the same in either case, but I just need some help on how I organise it ie. into a Statement Of Evidence referring to my Witness Statement, or just into my Witness Statement.
Does anybody have any ideas as to which one of these the judge intends me to do?

You just do a witness statement setting out the information you have been told to give.
You also prepare a list of all relevant documents (including your bank statements and bank's tems and conditions) and put all the documents in a pile in the order they appear on the list and you refer to the list and bundle of documents in your witness statement when you mention something on the documents that you want the judge to look at.
When the Liberals and Conservatives were in opposition they both agreed that banks should pay back high bank charges to customers. Nothing seems to of happened since they came into power as a coalition. PPI insurance has been sorted now they should turn their attention to bank charges and help customers get exorbitant charges refunded.
