I have recieved my statements and wrote out prelim letter do i send a copy of statements to the bank outlining charges to my account

Just send a schedule of charges: type of charge, amount ,date.
Use the Simple s/s here:

Save it(for use at court), delete the last 2 columns (8% bits) and print out
