I used to have a providian national credit card a few years ago but don't have their address and i have also lost my account number, can i still claim from them and does anyone have their address?

When Barclays took over my Providian card was replaced with a Monument credit card and I had a standard letter stating that Providian is now called (Barclays Bank Plc t/a) Monument. On my most recent statement (August 2006) the addresses are :-
