What goes into the 'In the' box or is it for court use?

Want some advice on completing the form

Who do I put as the Defendant? My Local Branch of Lloydsicon or what?

I know what to put in the Particulars, I have seen the prepared one but what do I put for the Brief Details?

Value! do I put a Breakdownicon or what?

    Help Appreciated thanx

Hi Andy, put your local county courticon name in the 'in the' box, your name and address as the claimant. I used Lloydsicon TSB Bank Plc, 25 Gresham St, London, EC2V 7HN (their head office) in the defandant bit, but I don't think it makes much difference.
Brief details... Claimant is claiming return of charges applied by defendant, plus costs etc..

value... charges of £XXXX plus interesticon of £XXX plus DPA fee £10 then put total in the 'total amount' box.

I filled in the N1 and did the above, (am now at the waiting stage after sending in my AQ)
    Good luck!

As Brief Details of Claim I put
'Claimant is claiming return of charges applied by defendant in respect of purported breaches of contract, plus costs'
    Does that look ok?
