There's a "step-by-step" bit somewhere on this site, don't have the link atm, but it's easy to find and very helpful! (I printed it out for reading!)
Also, it's best to read the FAQs, to know what you're doing.
The first letter you send , to find out what they charged you, is the DPA (in the "library" here on the site).
