The applicant selected the PPI on the app form when "upgrading" his card. He had to tick the box - it wasn't an "opt out" one. He does have an existing medical condition, and he had claimed on the PPI beforehand for unemployment.
He does not recall if there were any T&Cs attached to the application regarding PPI or any warnings.
