hope this is going in the right place. sent dpa letter out and recieved back a form to fill in. having a little trouble understanding section 4

given this form asks on the front page for name address creditcard number date of birth and telephone number and on page 2 for who the data you are requesting is about. my questions are do i have to repeat everything in the box provided? and is this the place i ask about manual intervention? or can i just print off a DPA letter and refer them to it?
sometimes can get bit confused given it half sounds like do i know which filing cabinet my records are kept in
