Default Whats my next step with Lloyds? Any help?

Some of you may know the problems I have been having with Lloyds.
I dont know what my next step is.
I have signed the slip for my Subject Access Request, just altered it a little . However with my CCA they cashed the cheque 2 weeks ago, and its been 4 weeks since they recieved the request. Now, I dont know whether to send an 'account in dispute/default' letter as although I have asked for the orig CCA, the slimy buggers got me to sign another ca last year when they was changing cc over to airmiles mastercard (said i HAD to sign this to transfer balance over otherwise I couldnt use my card). So....if they havent provided me with the orig cca whats my next step. Because if I stop payments etc, they could say well its a different account now...But I really want to get the lying cheating scum.
Also my complaint with regards to the ppi has been going on since feb. In may I sent them a letter listing things that I want to see as their proof I agreed to ppi, as they had previously told me that they had evidence (didnt say what tho, funny that!). And lo and behold I have heard nothing!
So now I want to hit them from all angles. FSA, information commisioner and all that. So who can I report them to, and have you got contacts details for them?

Hi spam, yes they did change. Although I have cca'd the original account number as it was that one the insurance was added to.
I was thinking about cca'ing the new account numbers, but I have a feeling they will have that cca as the new cards were sent to me a week or 2 later, although I never activated them.
Just a bit annoying as its only now I realise it was a new agreement i was signing,even though they never explained that to me!
