CCA request - help please

Guys,

Can someone point me in the direction of a template I can use for a CCA request please?

Any help here would be really really really appreciated.

what i do is say I am writing to you by means FIRST CLASS recorded delivery REF xx 1234 5678 AGB ( any interested party can check the successful delivery of this communication by visiting the royal mail website).

This way if you lose the slip you yourself can check it out and do a screenprint. In reality the updating is vey slow but often you get a signature and it's a just in case !!!.
